Hoping that I can receive some advice. Just received a diagnosis of ankle OCD.
Sprained my ankle on August 3, 2013, and it never healed.
My life totally derailed, have not been back to work.
My orthopedist has ordered a foot immobilizing boot, which I am to wear for 6 weeks.
Question - at my age (64) would this boot be unlikely to work, and surgery probably necessary.
Thanks in advance for any information.

Hi, I was 58 when I had mine done (OCD repair). This is a link to a previous post I made that explains what I had done (MF surgery). I have my doubts on a boot working, the cartilage won't come back without drilling.

hi Patdob, welcome to the site, glad you found us.
First, sorry to hear about your ocd.

As for your questions.. doctors order the boot in order to try conservative measure first, but if the ankle has not healed on its own then why would a boot help? I went over a year doing "conservative' Measures and frankly they were a waste of time in actually Healing the ocd. What the boot might help with is letting you live with the ocd in less pain but you seem to be in bad pain if you can't even work.

OCD , as you may have learned, do not heal on their own. Cartilage does not grow back. The best you can hope for is for the ankle to either stay at its current health but realistically it will get worse over time.

In my opinion, surgery is probably necessary. I would read up on the various surgery experiences on this forum. Denovo seems to be a popular one lately.

this journal article (literature review) from 2009, reviewing some existing studies, suggests that 45% - 53% of symptomatic osteochondral lesions of the talus improved with conservative treatment. (small sample sizes, however, and old studies). Improvement here is referring to symptoms, NOT actual repair of the defect, cartilage fill, or changes to MRIs.

will the cartilage actually repair itself in a grown adult? highly unlikely, as cartilage lacks the blood supply to repair itself the way other tissues do. in children/adolescents with this injury, cartilage regeneration would still be a possibility. (in the future, with stem cells, adults might also regenerate cartilage. veterinary medicine is on to this already.)

will conservative treatment help with pain relief, etc? possibly. and many surgeons take the attitude that asymptomatic lesions are best left alone. if it were me, I would try the conservative measure first. might or might now help, but really, what is there to lose?

Hi Patdob,

I am a newly diagnosed as well with an ankle sprain a few days after yours last Aug. 2013
(and getting some good info and encouragement from this forum).

I am probably too early in my OCD education to be offering advice, but what the others said,
sounds like good advice (OK to try the boot, but start researching surgical options).

I would also emphasize getting other Dr. opinions as well.

How is the boot working? Is your ankle feeling any better?
