Anyone have a failed Denovo?

Hi everyone- I'd love to talk to someone who has had the Denovo on the talus and had it fail. I had my first one placed in July 2011 and had only a few months of pain relief and then it worsened. The graft had not adhered, they think because the surgeon drilled into the bone first (microfracture) forming a kind of blister underneath. When I moved to Denver, I got a new surgeon and he put in another Denovo. I had pain relief from September 2012 to about July 2013. I was running short distances and working out and the pain came back, not as bad as the first time though. I backed off the running, then rowing started to hurt it a lot.. so I stopped that and went to yoga... which still made it hurt. My surgeon wants to scope it again and I am trying to decide if it is worth the $2000 out of pocket it will cost when I've already spent about $5000 over the last 5 years on it. Is it just going to hurt forever? Both surgeons said they had never seen one fail... there must be someone else out there! Please feel free to email me- [email protected].

I am 4 weeks after my denovo surgery, and am early in my rehab phase, so can't  comment based on direct experience. But I read a lot about denovo before my surgery, and did not come across many reports of a failed denovo. I did read about people having trouble with overgrowth, and one report of a graft delaminating where the denovo was put on top of a deep defect, without a bone graft being done to fill the defect.

How deep was your defect?  Based on by reading, 5-7mm seems to be the threshold depth for a bone graft. How long did you wait before returning to activity?  I'm planning on 6 months before I'll be able to walk for reasonable distances, and 12-18 months before I'm able to walk significant distances eg 5 miles +.  I don't plan on ever running again, as I spend close to a year before surgery having difficulty walking even a block, and don't want my ankle to degrade again.

I have had a failed De Novo transplant. I hit the two year mark back in July 2015. It started hurting again at about 1.5 years out from the surgery, and has continued to get worse and worse. I'm currently in chronic pain and feeling pretty hopeless.

I am very curious how you are feeling now and what step you've taken since you posted this to help alleviate some of the pain.

I initially had a microfracture in 2012 and have had several other associated surgeries, all on my left ankle. Sounds like we are in the same boat. I'd love to connect.

Henry. I'm glad I found you. My Denovo 2 year mark will be next month. I read your other post about having sharp pain. I have the same thing, especially after being active at all. And by active, I mean walking more steps than usual.

My mircrofracture gave me trouble but it allowed me to have about 10 decent years of playing sports, which is more than I can say for denovo. It seems that most people have a positive experience with it, but for whatever reason, I was not one of those people.

I had a heart to heart with my doctor last week. He's not big on the experimental/new procedures just because the data is so sparce. He's inclined to do another mircrofracture just because that's worked for me in the past. I actually have it scheduled for next month, but have a lot of mixed feelings about it.

I heard from Katy about 5  months ago, and she hadn't had any other surgeries at that point.

Hi, I have had failed DeNovo.

My cartilage was torn in May of 2013, and I got DeNovo in August of 2014. About 3 or 4 months later it felt great. Needless to say, since I am very active and am only 20 years young, I thought it was fixed. I instantly returned to a very active lifestyle including skating, hiking and biking. All was well until around July of this year, when I went on a backpacking trip followed by trying to get into serious cycling. Then the pain came back. Gradually at first. But, after a month of increasing pain, it got to the worst it's ever been. Way worse than even before surgery. Ever since then I haven't traveled more than a mile each day, and have been alternating between a boot and crutches. Recently, I've been using only crutches because the pain is too much from walking in a boot. I'm scheduled to see a Dr. Bob Baravarian in LA next week and I'm hoping that the meeting will renew some hope. Dr. Baravarian has published a lot of articles in Podiatry, and the success rate of his patients looks promising. I guess the only reason I'm writing this post is to see what people's experiences have been with different treatment options? No way in hell am I going to get DeNovo again. It doesn't make sense why I would want to put someone else's cells in my body because of the high risk of my body rejecting it (which it did). I have done extensive research on ACI (Autologous Chondrocyte Implantation), Regennex (bone marrow aspirate stem cell treatment), and the use of collagen scaffolding alongside these treatments. I also recently heard about HGH, or IAGH treatments, which seem very iffy. Some people say their great, but I find it weird that I haven't heard more about it from scholarly sources. My question is, what's worked for you? if anything? I am going to ask Dr. Baravarian about all this next week, but I want to hear it from the patients point of view. One thing I've learned from the first failure is that I didn't give it enough time. This time, whatever my treatment, I am going to give it a minimum of 6 months non weight bearing, and at least a year of taking it easy. Before I return to activity or anything like that I will make sure to have MRI and arthroscopic assessment to verify that I am fully healed. I have potentially 60 or more years of life, and I think that it makes a lot more sense to use crutches for a while (year or so) while I'm 20 than it does to live in pain for ~ 60 years. Please tell me about your experience! I would love to hear your success stories, or advice taken from not so successful stories. This problem is getting in the way of my whole life as a student and I am going to focus all of my energy to fixing it.
