original injury and size of ocd(s):OCD defect located on taler dome, I don't have the information describing it's exact size, but the doctor said it was quite large (I will update when I find out what size it was)

description of current injury status: Sprinting, jumping, playing basketball, but still slightly conservative on pushing things.
any other info: The progress I've made really is fantastic. While my ankle does flare up on occasion, it largely never bothers me to the point where I feel the need to stop for any given task. As far as how far Iv'e pushed it goes, I literally just wrecked my high school sister in a 100 meter dead sprint, and she's a state placed runner for her track team-so all in all, (from both a self esteem and physical standpoint) things look peachy. On a side note, I am a little nervous regarding how well the "denovo" procedure will stand for in the long term. I mean, there really is not much information relating to what I may expect with this procedure at any point down the road, so I guess from a medical standpoint, I'm in some sort of "no mans land". Since my doctor would only warrant a "post-procedure" MRI if problematic symptoms occurred, all I can really do is slowly increase my activity (particularly jumping) while being both conservative and worry of any pain or instability.

Great idea, and thank you and thank RB for sharing info.

I was wondering if we might add to your list of suggested questions:   Surgeon performing the procedure, and age at the time of the procedure? 

Thanks!   I'm still a few days shy of 12 weeks, so premature for me to share info.  But will certainly do so after a year!

Time for an update, it's been some time since I visited the site. I had MF for a large OCD in October 2011 (6.5 years ago at this point). Ankle is still very good, rarely have pain or stiffness. I am very active, hike the trails several times a week with my dog. I always avoid impact type activities, I walk and hike fast, but never run or jump. Just my 2 cents, but I feel it was completely successful, it changed my life and I think the replacement cartilage will continue to last.

I've had a couple surgeries so I'm just going to give a summary of their success (or lack thereof) here. I put the whole long story here: http://osteochondraldefect.net/forum/index.php?topic=371.0

Surgery #1 (2008)
original injury and size of ocd(s): Repetitive ankle sprains. Not sure about OCD size. Initially not very big.
type of operation: microfracture (MF)
patient age at operation: 29
performing surgeon: Marc Rankin
surgery outcome: Good outcome on OCD repair. Climbed Mt Kilimanjaro about 6 mos. later and was training up for a marathon 1 yr later. Never repaired ankle instability, though, and re-injured it on a hike about 22 months later.

Surgery #2 (2012)
original injury and size of ocd(s): Repetitive ankle sprains. Not sure about OCD size.
type of operation: MF + modified Brostrom-Gould (B-G).
patient age at operation: 33
performing surgeon: Daniel Lahr
surgery outcome: Mostly successful B-G. Stopping rolling ankle. However, some continued scarring and lack of motion near the ligament repair site. OCD repair failed. Over the next ~6 years, the ligament began to stretch out more to the point where there was some continued instability and needed another ligament repair 6 yrs later in 2018.

Surgery #3 (2013)
original injury and size of ocd(s): Not sure about OCD size.
type of operation: MF w Denovo
patient age at operation: 34
performing surgeon: Daniel Lahr
surgery outcome: MF with Denovo. Some improvement but overall not a great outcome. Significant pain and inflammation in ankle. Unable to run. Pain when standing for >30 min or walking >1 min. Reduced range of motion. Very reduced quality of life. Slowly deteriorated over time, unless complete failure ~5 yrs post surgery and unable to walk.

Surgery #4 (2018)
original injury and size of ocd(s): At this point in time, had a 1 cm shoulder lesion on the medial talar dome, a small kissing lesion, and a small lesion on the lateral talar dome. Also some modest instability in the ankle joint that seemed to be contributing to the stress on the cartilage and lesions.
type of operation: OATS (autograft from knee), ligament repair, plus Concentrated Bone Marrow Aspirate (CBMAC) from my hip and Biocartilage to help repair the smaller lesions
patient age at operation: 39
performing surgeon: John G. Kennedy
surgery outcome: TBD -- only 1 week post-surgery so far. I will post updates going forward.

type of operation: debridement and talar drilling
patient age at operation: 25
time since operation: 13 months
performing surgeon: Dr. Faux Utah County, UT
description of current injury status: regular clicking from walking, minor physical impediment on dorsiflexion, no pain on walking or running, minor swelling day to day

description of current injury status: OCD returned in 2018
any other info: No pain following first surgery, successful PT, returned to normal

Second Round
original injury and size of ocd(s): 2018 OCD returns, right ankle, 19.5 x 7 mm
type of operation: OATS
patient age at operation: mid 30s
time since operation: 2 years, 8 months

Surgery #1 (2020)
original injury and size of ocd(s): Ankle torqued in BJJ.  Osteochondral defect of the medial talar dome.  Cystic area measured 22mm x 15mm.
type of operation: Bone graft - Minor, Subchondroplasty, Ankle Arthroscopy
patient age at operation: 39
performing surgeon: Bryan Levay
surgery outcome: About 3-4 months out I was noticing pain similar to what I was experiencing pre-surgery.  Performing surgeon ended up leaving the practice shortly after my surgery and I was transferred to another Dr. (Robert Toomey) who was responsible for my recovery.  I was told that the pain was normal and that after a year or two it should clear up.  I was not happy with the practice after repeated visits when my concerns were dismissed.  The pain, although sporadic, did not give me the confidence that it had been fixed.  After 2 years I visited a new practice and was advised to get an MRI.  The new MRI revealed that the surgery was not successful and I was advised to get surgery once again.

Surgery #2 (2022)
original injury and size of ocd(s): Osteochondral defect of the medial talar dome.  Cystic area measured 22mm x 15mm.
type of operation: Denovo
patient age at operation: 41
performing surgeon: Robert Hallivis
surgery outcome: Dr. Robert Hallivis recommended Denovo as it was the least invasive option.  I got a second opinion from Dr. George Kartalian who agreed.  I proceeded with the surgery and recovery went really well but things are not 100%.  Biggest discomfort I have is that my ankle has early onset arthritis.  It bothers me the most when I go from a resting position to a standing.  Sitting to standing Its not that bad even though I limp a bit.  However, when I wake up in the morning I have to gradually shift weight back and forth between my good ankle and bad in order for it to be comfortable enough to walk.  It doesnt take long, maybe 30 seconds to a minute.  I do still experience a sharp pain here and there but mostly its just the soreness and stiffness associated with the arthritis.  The Dr said it may get better over time and if it continues to be a problem he could prescribe anti-inflammatories.  Instead of relying on that Ive cleaned up my diet by introducing anti-inflammatory foods and supplements while cutting out things like alcohol.  Although Im cleared to return to normal activity, I choose to avoid high-impact exercise.  Weightlifting hasnt been an issue but Im being smart here and not looking to set any records.  Acupuncture has helped as it has assisted in reducing the duration of the soreness I experience from going to a resting to standing position.  It used to be minutes before I would find comfort again but I feel that the acupuncture along with the other lifestyle changes Ive made have all been positive contributions.  Another thing I noticed, which was really strange, is that most of the sharp pains I experienced were when I was out walking.  However, inside the home I didnt experience any of these pains.  Since Im barefoot at home and wearing running shoes while out I figured that the shoes must be the source of the problem.  I switched to a new, expensive pair of running shoes and the problems persisted.  Then I thought maybe the inserts so I got custom orthotics which did not help.  I then decided to scrap the running shoes and inserts entirely and switch to minimalist shoes.  These have helped tremendously.  I still experience a sharp pain here and there but no where near the frequency as before.  So overall Im ok with the results from Denovo.  Im a little over a year out and still hoping things continue to improve.  If I had to throw a percentage of recovery around it I would say about 85%.
